Direct/Support Services
PathWays Support Services provide assessment, planning, coordination, monitoring, and evaluation of options and resources to meet an individual's specific needs. PathWays applies a combination of Community-based Case Management and Wraparound Services (not something that you “get”, it’s something you “do”); it’s a process, not a program or a service. Rather it is an unconditional commitment to team development, strengthening of family with outcome based interventions.

PathWays knows that stable, affordable housing is a key determinant of health. The house and neighborhood where one grows up impacts the health and longevity of one’s life. We provide resources that help build & maintain one’s most important assets (financial stability and your home).
PathWays promotes access to personal counseling and financial resources designed to stabilize living environments and provide the foundation for economic security.
